The Hospice Admission Nurse is responsible for admitting patients to the hospice service and palliative care service and developing appropriate plans of care. The hospice admission nurse is responsible for accurate and efficient completion of all admission components, including HIS and comprehensive assessment and establishment of the initial plan of care in keeping with best practice standards in addition to the development of the OASIS data set and orders for service for palliative care. Essential Duties and
Responsibilities:
Responsible for providing comprehensive assessment of patients, including holistic assessment of physical, psychosocial, spiritual, and cultural factors of the patient and family related to disease or injury, risk factors, safety issues, function, cognition, sensory status, ability to communicate and learn, and ability to accept and cope with illness, death and bereavement Responsible for accurate and timely completion and submission of HIS data set as part of the comprehensive assessment of the patient. Demonstrates timely and accurate computer documentation of patient/job information to facilitate the development of the OASIS data set, orders for service, creation of time sheets and other required patient/employment documents and reports. Responsible for utilization and integration of best practice standards in the assessment of patients and families and in development of patient-specific care plans. Responsible for the reconciliation of medications, initial medication assessment and teaching and establishment of the medication profile. Responsible for initiation of patient teaching and the provision of appropriate agency teaching materials and resources Responsible for establishing a plan of care, in conjunction with the patient and family, addressing all active problems and risks, and establishing goals for care that are accurate and measurable. Responsible for identifying the appropriate payer for services in keeping with payer qualifications and conditions, and for providing appropriate notification in situations of non-coverage. Responsible for the admission of patients to services in keeping with required productivity standards Participates in-service/continuing education as required Participates in the orientation of staff members to the agency and its policies and procedures. Communicates effectively and tactfully with patients, recognizing their age, cultural diversity, needs, abilities and physical condition. Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements. Performs related and other duties as required. Minimum

Experience:
Knowledge of State Regulations, Medicare, and Medicaid regulations is required. Experience in home health or acute care settingCertificates, Licenses, Registrations:
Current Connecticut Registered Nurse LicenseOther:
